Antibiotics for Delirium in Older adults with No clear Urinary Tract Infection

[2] Research Question/Objective:
Do adults (age 60 or older) with delirium and suspected infection benefit from taking antibiotics?
[3] Key Inclusion Criteria:
Age ≥ 60 and active delirium
[4] Key Exclusion Criteria:
Fever (temperature > 37.9C or > 100.2F) in the past 48 hours;
Signs of lower urinary tract infection symptoms (such as new dysuria) or upper urinary symptoms (such as costovertebral tenderness)
[5] Intervention:
Start/Continue antibiotics
Stop/No antibiotics
[6] Study Primary Outcome:
Delirium at day 7 or at day of hospital discharge, whichever is earliest.
BALANCE+
BALANCE+: A Platform Trial for Gram Negative Bloodstream Infections

[2] Research Question/Objective:
The goal of the BALANCE+ clinical trial is to transform random care to randomized care for patients with Gram negative bloodstream infections to inform best treatment approaches and optimize outcomes.
[3] Key Inclusion Criteria:
Positive blood culture with Gram negative (GN) bacterium
[4] Key Exclusion Criteria:
Patient's goals of care are for palliation with no active treatment;
Moribund patient, not expected to survive > 72 hours
[5] Intervention:
Follow-up blood culture
Beta lactams vs. no beta lactams

Domain-specific recruitment rate;
Domain-specific protocol adherence
